I hereby acknowledge this release from liability for accidental injury or illness which my child may incur as a result of participating in any physical activity. We hereby assume all risks connected therewith and consent to participate in this MVP program. We agree to disclose any physical limitations, disabilities, ailments, or impairments which may affect my child’s ability to participate.

I hereby acknowledge this media release granting MVP Sports and Training, LLC all ownership rights and the absolute and irrevocable right and permission to copyright, use and publish the recorded image and/or voice of my child obtained pursuant to this Consent and Release for the use of MVP promotions.

I hereby acknowledge I will recieve SMS updates and promotions from MVP Sports.
